Service Description

The follow-up appointment is delivered through a structured clinical process that may include: Review of assessment outcomes A collaborative discussion of the assessment findings, including diagnostic conclusions (where applicable), cognitive profile, functional impacts, and key themes identified during the assessment process. Clarification and discussion Opportunity for you (and/or your family, carer, or support person where relevant) to ask questions, seek clarification, and discuss any aspects of the assessment or report in more detail. Review of recommendations Detailed consideration of the recommendations provided in the assessment report, including practical application to daily life, education, employment, or support services. Goal setting and planning Discussion of priorities and next steps, including identification of goals, supports, and referrals that may be beneficial based on the assessment outcomes. Liaison and coordination (where consented) Where appropriate and with consent, discussion of communication with other professionals, schools, workplaces, or support services to support implementation of recommendations.
